Instructions:
The case continues application of the Root Cause Analysis (RCA) method and making
recommendations using what you’ve learned in this course, as well as your personal knowledge
and experiences.
1. Read the Harvard Business Case “Data Breach at EquiFax.”
2. Analyze the case, prepare and submit a written case study analysis using the Root Cause
Analysis (RCA) method and steps, and addressing the below issues and questions. The length
of a typical case study analysis paper should be within the range of 4-5 typed pages, doublespaced. If you would like to include additional information beyond the 4-5 page limit, you may
provide it as an exhibit or addendum to your basic paper
Issues to address:
1.Identification of relevant facts and issues
2.Accurate identification of the root cause
3.Development of alternative solutions (where appropriate) and discussion of the
advantages/disadvantages of those alternatives.
4.Recommendation of a solution/solutions to resolve the identified root cause; identification of
the human resources (HR) and financial concerns relevant to your recommended solution.
5.Cyber security lessons learned.
