Barriers to Obtaining Behavioral Health Care

Barriers to Obtaining Behavioral Health Care

As we go through this course, we will find that there are many barriers to obtaining behavioral health services.

Some of those barriers are financial in that many services are not 100% covered by insurance.

Some practitioners do not accept any insurance, only cash and the client must then submit the bill to the carrier, usually for partial reimbursement.

There are not enough practitioners in many places in America and the world. For example, there are 27 child psychiatrists per 100,000 children in New York State. https://www.aacap.org/aacap/Advocacy/Federal_and_State_Initiatives/Workforce_Maps/Home.aspx (Links to an external site.)

“Ghana has a population of approximately 28 million people, yet the country has an estimated 18 to 25 psychiatrists, up from 11 psychiatrists in 2011.1-3 Compared with the United States, which has 10.54 psychiatrists per 100,000 people (approximately 1 psychiatrist per 9,500 people), Ghana has .058 psychiatrists per 100,000 people (approximately 1 psychiatrist per 1.7 million people). (Smalls-Mantey, 2019).

There are approximately 8,300 practicing child and adolescent psychiatrists in the US and over 15 million children and adolescents in need of the special expertise of a child and adolescent psychiatrists (American Academy of Child & Adolescent Psychiatry, 2019) retrieved from: https://www.aacap.org/AACAP/Resources_for_Primary_Care/Workforce_Issues.aspx (Links to an external site.)

Stigma is a barrier to obtaining behavioral health care. Many cultures do not accept the concept of mental illness, believing instead that the person is possessed or is “cursed.”

Denial is a barrier to behavioral mental health services as in “I don’t have a drinking problem. I can quit anytime I want.”

Immigration status is a barrier to behavioral health services. Being undocumented in these times discourages individuals from seeking care as ICE, the immigration enforcement officials are now coming to courthouses and hospitals to remove those individuals who are lacking citizenship or tourist documentation. US citizen children are also failing to receive appropriate services as their parents are afraid to bring the child in for services for fear of family separation/deportation. Lastly, immigrant children are being denied mental health services after being traumatized by being separated from their families and detained indefinitely in strange surroundings.

Interrelationship between cost access quality in the delivery of health services

Interrelationship between cost access quality in the delivery of health services

Essentials of the U.S. Health Care System, Fifth EditionLeiyu Shi and Douglas SinghCase StudyCopyright © 2019 by Jones & Bartlett Learning, LLC, an Ascend Learning Company2Case ReportDuring the summer of 2018, Blue Cross and Blue Shield, the Texas-based health insurance behemoth, entered an ACO relationship with Baylor Scott & White Health, the largest nonprofit health system in Texas. Under the agreement, in year one Baylor Scott & White would provide high-quality care to upwards of 140,000 Blue Cross and Blue Shield members, establishing it as the most substantial value-based care agreement with a commercial insurance company in the United States (Rege, 2018). Improvements in health outcomes for those with chronic diseases such as diabetes, hypertension, and obesity can build patients’ confidence in the ACO model resulting from increased quality of life, leading to higher levels of customer satisfaction in the health system. DiscussionThe shift from healthcare delivery based on patient demand to a more proactive approach will require a complete commitment from all healthcare stakeholders. Due to its ACO alignment, Baylor Scott & White Health had over 1 million lives under value-based care arrangements, and stakeholders believe that their patients will receive high-value, coordinated, and quality care (Rege, 2018). Designing, implementing, coordinating, and managing the delivery of health services can increase care and oversight across the entire continuum of patient care (Wexler et al., 2014). These tighter controls at the point of service will lower costs across the entire system. Increasing emphasis on disease prevention will directly affect the rate of chronic conditions and traditional high-cost illnesses. Moreover, financial risk pools will create economies of scale as younger and healthier patients begin to purchase insurance plan resulting from the ACA mandates. Health policies and reforms such as the ACA demonstrate the government’s meaningful efforts to reduce costs and increase access to and quality of care. These types of Essentials of the U.S. Health Care System, Fifth EditionLeiyu Shi and Douglas SinghCase StudyCopyright © 2019 by Jones & Bartlett Learning, LLC, an Ascend Learning Company3national standards are enforceable by government agencies with far-reaching authority. The lower cost of medical procedures coupled with reduced reimbursement rates from Medicare and Medicaid will fundamentally alter health care delivery in the United States.
Questions
1. Discuss the interrelationship between cost, access, and quality in the delivery of health services. Briefly describe the impact of each measure
on positive patient health outcomes.
2. Under the ACA, describe a scenario of how medical providers and healthcare professionals work together to enhance the quality of health
initiatives, interventions, or outcomes.
3. In your opinion, what role will ACOs play in the future environment of value-based health services delivery? Explain your answer by
integrating an example of how an ACO can either reduce costs, increase patient satisfaction, or improve health outcomes

Conference presentation (Diabetes New Zealand)

Conference presentation (Diabetes New Zealand)

This assignment asks you to analyse professional, lay, and personal knowledge around a health-related topic or health promotion/health education initiative of your choice. You should make this choice during the first 3 weeks of the course. There two parts to the assignment – an aural presentation, and a written piece.
Part 1: Written Case Study
Describe the dimensions of an issue or initiative (e.g. Anti-bullying campaign; The MacDonalds ‘Scale for Good programme’; B4 School Health check; Healthy Families New Zealand; Wai Puna – Water safety; The Well-being Budget; The Childhood Obesity Plan; Te Tai Timu Trust’s initiative… the list goes on). Tell us what it comprises and what its features are.
What lies beneath? Examine the professional, lay and political ideas underpinning the issue/campaign you have chosen. What kinds of points of view about people, health and what is important in life, underpin it?
